• Pricing
  Are Prices are displayed in Australia Dollars and they are subject to change without notice. For home visits, additional fees may apply depending on location and time.

• Consent

  By attending a Home Visit or Online Physiotherapy session, you are providing consent of being assessed and treated. Your physiotherapist will ask you for your consent prior to starting treatment.

• Privacy

• Handling Of Personal Information.

  Revival Body & Mind collects details for a number of reasons, including treatment notes, marketing, patients history, personal communication and management. By attending a Session with one of our Physiotherapists, you give consent to Revival Body and Mind to use your details in a respectful manner, under the Australian privacy principles, Spam act and Privacy Act.

• Data Security
  Revival Body & Mind holds the data of its clients to an outstanding level of security. We use softwares with high records of data security to maintain your records safely. However, please contact Revival Body & Mind if you wish to you have your data removed from our system.

• Payment
  Payment is to be made at time of consultation or over the phone. Revival Body & Mind accepts various forms of payment such as cash, credit card and private insurance card from approved insurers. Revival Body and Mind reserve the right to charge a credit card fee.

• Cancellation and Late Appointments
  Revival Body & Mind requires a minimum of 24 hrs notice if you wish to reschedule or cancel your appointment. This will give the opportunity to provide therapy to our clients who are on our waiting list.

  If you decide to cancel an appointment or reschedule without providing 24 hrs notice, a fee of $95 will be charged to you. If you are running late for an appointment, depending on therapist availability, you may be only able to receive the outstanding time of your appointment for treatment.

• Guarantee
  All our therapists are highly trained in their field and will make every attempt to assist your treatment plan and goals. However, Revival Body & Mind makes no guarantee of effect of treatment. In the event that the outcome of therapy is not as planned, the client agrees that the service and has been provided, and no refund should apply.
